What is Root Cause Analysis?

Root cause analysis (RCA) is a structured problem-solving method for identifying the underlying cause of a failure or issue — not just its symptoms. Rather than applying a quick fix that may let the problem recur, RCA traces the causal chain back to the fundamental source.
